	Assumption and Acknowledgment of Risk: I understand that participation in the LakeGeorge.com Elvis Festival, whether as a volunteer, participant, performer or competitor, is for my enjoyment, and my decision to participate was made freely. I recognize, and acknowledge, that participation in this festival may involve risks of injury including the possibility of death. I voluntarily agree and expressly assume all risks of injury or death that may result from my participation in any of these activities or which may relate in any way to the use of the facilities provided by the LakeGeorge.com Elvis Festival. As a condition of being permitted to participate in event, and any use of equipment provided by the LakeGeorge.com Elvis Festival I assume all the foregoing risks and accept personal responsibility for the damages to me following such injury, permanent disability, or death as a result of my participation in these activities and any damages or injuries which may occur as a result of my participation in these activities or use of the equipment provided for my use. I understand that this event is being filmed and photographed on behalf of the LakeGeorge.com Elvis Festival and I consent to be filmed and agree that the LakeGeorge.com Elvis Festival and Elvis Presley Enterprises, Inc. or any of their assignees, may use and grant the right to use my voice, name and filmed and/or recorded likeness without limitations or restrictions of any kind and without payment to me of any kind for all purposes (including in any production or any advertising or promotions). I agree to release the LakeGeorge.com Elvis Festival, Elvis Presley Enterprise, Inc, its affiliates and sponsors from all liability in connection therewith. I understand and agree that any claim, which I may at any time bring against the LakeGeorge.com Elvis Festival, its owners, officers employees or agents, shall be brought in the State or Federal Court in the State of New York, Warren County, and that no claim or action shall be brought in any other jurisdiction. I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL THE TERMS AND CONDITIONS ABOVE AND AGREE TO SIGN OF MY OWN FREE WILL.
